smartForm is an online form builder operated by SmartApz LLC, a Michigan limited liability company (“SmartApz”, “we”, “us”). This policy explains what we collect when you use smartForm at smartapz.com/smartform and how we use it.
Two kinds of people use smartForm
Form owners create an account and build forms. Respondents fill in those forms. For responses, the form owner decides what to ask and what to do with the answers; we store and process responses on the owner’s behalf. If you filled in a form and have a question about your answers, contact the person or organization that sent you the form.
What we collect
- Account details: your email address, optional name, and a securely hashed password (we never store your password itself).
- Your forms: titles, fields, settings and the responses they receive, including any uploaded files and signatures.
- Technical data: for each response we keep a one-way hash of the sender’s IP address and their browser’s user-agent, to fight spam and abuse. Our servers also keep short-lived access logs.
- Payment details: if you upgrade, payments are handled by Stripe. We receive your customer and subscription IDs and status, not your card number.
How we use it
- To run the service: show your forms, collect responses, send you email alerts for new responses, and let you view, export and delete them.
- To keep it safe: rate limits, spam checks and fixing problems.
- To email you about your account (for example password resets, plan limits or billing). We don’t send marketing email without your permission.
We do not sell personal information and we do not use form responses for advertising or to train AI models.
Who we share it with
Only service providers that help us run smartForm: our hosting provider (DigitalOcean, servers in the United States), our email delivery provider (Brevo) for alerts and account emails, and Stripe for payments. They may only use the data to provide their service to us. We may also disclose information if the law requires it.
Cookies
We use one essential cookie to keep you logged in. Public forms set no cookies. We don’t use advertising or tracking cookies on smartForm.
Keeping and deleting data
Forms and responses stay until you delete them. Deleting a response or a form removes it and its files from our servers right away (backups roll over within 30 days). To delete your whole account, email info@smartapz.com from the address on the account.
Security
Connections are encrypted with HTTPS, passwords are hashed, and uploaded files are stored outside the public website and are only available to the form owner when logged in. No system is perfectly secure, so please don’t collect highly sensitive information (such as full card numbers or medical records) with smartForm.
Your rights
You can access, correct, export (CSV) and delete your data in the app, or ask us at info@smartapz.com. Depending on where you live you may have further rights, and we will honor them.
Children
smartForm isn’t meant for children under 13 to create accounts.
Changes
If we change this policy we will update the date above, and tell account holders by email about important changes.
